Queens Public Library requires individuals to submit relevant information at various stages of volunteer service.

Before You Volunteer...

Parental Consent Form

Applicants under eighteen (18) must obtain parent/guardian consent before volunteering with Queens Public Library.  You must download, print, sign, and upload with original handwritten signature.

Application for Department of Education-issued Working Papers

Applicants under eighteen (18) must obtain and submit valid working papers before volunteering with Queens Public Library.  The application should be returned/submitted to your school to receive the working papers. Call your school's main office to inquire. If they are not available, you can also:  

  1. Reach out to the Family Support Coordinator for your school. To find out who your Family Support Coordinator (Find your school district here

  2. Find out who your Family Support Coordinator by scrolling down to the appropriate District OR reach out to New York City Department of Education's District Family Advocates or call 718-935-2000
